Beneficiary designation

Q: My attorney wants me to use the designation of beneficiary form she has provided in my life insurance policy. How do I attach that form to my online application for coverage.

A: Write “see attached” when filling out the online application. Then fax the written beneficiary designation to (866) 271-5344 or mail to PO Box 102 Narberth PA 19072, Attn. Tony Novak. This procedure is commonly used when an attorney has established multiple trust accounts that are to receive proceeds of life insurance and it is important that the beneficiary is accurately named.
