Transferring health savings account

Q: I want to transfer my HSA from
Assurant to First American Bank, but there's no on-line form to
download and they don't respond to my emails and there's no phone
number for them on the application.
A: An online account enrollment
form is available at
http://www.healthsavingsaccount-hsa.com/hsaacounts.htm . Mail a printed
copy of the manually signed application with your initial deposit (or
account transfer request).
